Connecting Patients to Virtual Care Across Central Ontario
Empower Health provided the digital infrastructure for same-day virtual appointment booking serving more than four million people across Ontario's Central Region. Designed with local partners, the platform offered an easy-to-use experience in multiple languages and supported public access to care year-round.
In a 2025 pilot, patients without a primary care provider, while booking virtual urgent care, could choose from 16 local interprofessional care teams accepting new patients. After their virtual visit, each patient was referred directly to their chosen clinic for ongoing care.
This project shows how Ontario-made digital tools can simplify access, improve coordination between urgent and primary care, and support regional attachment goals — all while making care easier to navigate for patients and providers.
